Volunteer Information
Thank you for your interest in volunteering. To ensure the safety of our students and to be in compliance with Washington State Law, we ask that you following the directions below:
- VISTAS Volunteer Application Form (4 pages)
- Page 1 -Instructions
- Page 2 - Check box for VISTAS volunteer in upper left corner and complete all contact information
- Page 3 - Provide contact information for two references and indicate days and times available to volunteer
- Page 4 - Indicate school preference and sign and date bottom
- Volunteer Disclosure Form
- Answer ALL questions
- If you anser "Yes" to any question, a written explanation is required; attach a separate sheet to the back of the application packet.
- Sign and date at bottom
- Washington State Patrol Form (do NOT send to Olympia)
- Complete section C (social security number is not needed)
- Compoete section D (finger prints not needed)
- Copy of Photo ID needed
- Bring a valid photo ID with birth date to orientation. Orientation will be in late September. You will be notified of the time and exact date.
